Our services include assistance with daily activities, light housekeeping, respite care, essential transportation, and other non-skilled nursing tasks to promote independence and wellbeing.

Medicaid In-Home Care
Medicaid (Traditional/State Plan)
InHome Care Coverage Medicaid is a government-funded program that helps low-income individuals access healthcare, including in-home care services like personal care and skilled nursing. Each state manages its own Medicaid program with federal support, determining eligibility, services, and providers.
How It Works
Covers Home Care Services – Pays for personal care, skilled nursing, and homemaker services. State-Managed – Each state decides how much care is provided, who qualifies, and how services are delivered.
Available Through Medicaid or Waiver Programs – Some individuals may qualify for additional support through Home and CommunityBased Services (HCBS) waivers.

Consumer Directed Services
CDS
Consumer Directed Services (CDS) put you in charge of your own care, allowing you to manage your services and supports in a way that best fits your needs. Through this program, eligible participants have the freedom to hire, train, and supervise their own personal care attendants (PCAs), providing greater flexibility, choice, and independence in daily care.
With CDS, you can:
Select and manage your own caregiver – Choose someone you trust to assist with your personal care needs. Customize your care – Ensure services align with your personal preferences and lifestyle. Maintain independence – Stay in control of decisions that affect your well-being.
Participants also receive support from a Fiscal/Employer Agent (F/EA) to handle payroll, taxes, and administrative tasks, ensuring compliance with Medicaid regulations. This allows you to focus on receiving quality care without the burden of managing complex paperwork.

Once approved for a VA home care program, Choice Care will deliver high-quality, compassionate care and companionship tailored to your needs.
We offer support through several veteran in-home care programs, including:

Managed Care
Medicaid Managed Care
Coordinated In-Home Care Medicaid Managed Care provides the same essential in-home care services as traditional Medicaid but is managed by a private insurance company on the state’s behalf. These Managed Care Organizations (MCOs), such as Aetna or UnitedHealthcare, help coordinate and streamline your care.
How It Works
✔ Still Medicaid, But Managed Privately – Your benefits are administered by an MCO instead of the state.
✔ Coordinated Care – A care coordinator helps set up your home care, approves service hours, and arranges providers.
✔ Potential Extra Benefits – Some MCOs offer additional services, more flexibility, or enhanced case management.
We are proud to accept Missouri State Medicaid as well as Community Care Network coverage through the Department of Veterans Affairs (VA). In addition, for individuals who may not have insurance coverage or prefer an alternative option, we also welcome Private Pay arrangements. Our goal is to make our services accessible and affordable for as many individuals as possible, ensuring that everyone receives the quality care and support they deserve.
